Status

Covered with conditions. Covered from approved providers: occupational, behavioral, physical, speech and audiology therapy.

Conditions

Educational services and therapies, including occupational, behavioral, physical, speech-language and audiology therapies, from approved providers through the closed Marketplace. Assistive technology and equipment or supplies needed to accommodate a student with a disability are also covered, including rental. Out-of-pocket therapy can be reimbursed only if the student has a documented disability or specialized need and no approved provider within 30 miles offers a substantially similar service.

Limits

Specialized seating for students with disabilities over $1,000 needs disability documentation.

Hope portal Marketplace (services) or TheoPay (goods), Reimbursement (listed circumstances only)

Documentation

Marketplace order; for reimbursement, documentation of the disability or need and an itemized receipt within 90 days.

Not allowed

Medications, vitamins and supplements; reimbursement because the family prefers a provider that is not approved.
